2025 Sports Physicals

• No appointment needed.
• Immunizations will not be given.
• Cash or check only will be accepted.
• Please make checks payable to Sarah Bush Lincoln Fayette County Hospital
• Insurance will not be billed, nor will insurance co-pays be collected.
• Parent or legal guardian must present and complete the medical history forms.
• These physicals DO NOT meet the kindergarten, 6th or 9th grade physical requirement.
• All fees collected will be donated the school’s sports booster clubs in Bond, Fayette, and Marion counties

HISTORY FORM

All physicals take place at the SBL FCH Express Care Walk-In Clinic Saturday, July 19  9 am - 2 pm Tuesday, July 22 2 pm - 6 pm Saturday, August 2 9 am - 2 pm Tuesday, August 5 2 pm - 6 pm
